Weight Management & Diabetes Prevention
The Weight Management & Diabetes Prevention Program (DPP) is regular in-person small group meetings (1-on-1 virtual meetings available upon request) designed to discuss topics including exercise, nutrition, stress, sleep, and many other typical challenges in a weight loss journey. Sessions are designed to help individuals lose weight and even prevent or delay Type 2 Diabetes. Healthletix Lifestyle Coaches, all certified by the Centers for Disease Control (CDC), will hold regular sessions teaching problem solving skills and giving members tools for lifestyle change to help you reach your wellness goals.

The Healthletix Weight Management & Diabetes Prevention Program is a 12 month commitment to a program that focuses on true lifestyle change. This is not a short term diet plan. The ideal participant has a strong will for change and is someone who wants to make lasting changes to improve their current and future quality of life.
Sessions 1 – 16 = Weekly Meetings
Sessions 17 – 20 = Biweekly Meetings
Sessions 20 – 26 = Monthly Meetings
Weekly body weight
Quarterly BodPod Body Composition Analysis & Resting Metabolic Rate Testing
Weight Loss Program Participation Requirements:
*No program requirements.
Will work with and submit progress reports to your primary care physician, as necessary.
DPP Participation Requirements:
Must be ≥18 years old with a BMI ≥ 25 kg/m² (≥23 kg/ m², if Asian)
Recent blood test (within past year): Fasting plasma glucose of 100-125 (110-125 for Medicare Participants); Plasma glucose 2 hr after 75 gm glucose load of 140-199 mg/dl; A1C of 5.7-6.4; Clinically diagnosed Gestational Diabetes mellitus during previous pregnancy (may be self-reported)
Medicare Participants: No diagnosis of End Stage Renal Disease (ESRD); Blood test required
Diabetes 180 Participation Requirements:
The goals of this program are to lower A1C levels to <6.4 and control blood sugar fluctuations.
Recent blood test (within past year): Fasting plasma glucose of >125; Plasma glucose 2 hr after 75 gm glucose load of >199 mg/dl; A1C of >6.4
Clinically diagnosed with Diabetes
*Will submit regular progress reports to your primary care physician.*
All participants will need to have a “Pre-Participation Form” completed in order to begin the program. Act today and take advantage of this life-changing program. **Bloodwork within the past 12 months prior to Session 1 is strongly encouraged.**
